The non-time dependent regional networks of disease are organized with an hub & spoke model with the objective of optimizing the management of chronic conditions both on the hospital and on the local side . It elaborates diagnosis and treatment guidelines in order to ensure suitable times both for the first visit and follow-up with particular attention to the new diagnosed patient. On the other hand, a formal evaluation of their role in the management of non time-dependent diseases was not carried out, so far. This WP will conduct such an evaluation and will propose a new place of these networks in the context of primary health to guarantee the integrated management of clinical pathways with the general practitioner.
Nov 23-Mar 24. In recent months, work has been undertaken to survey the non-time-dependent networks for chronic diseases already formalized and active in the various Italian regional health systems with the aim of mapping the state of the art.
The survey on the regional networks on non-time-dependent diseases is initiated. The different regions authorities were contacted in order to obtain information on existing networks. The work has been focused on networks fo diabetes and cardiovascular diseases. Regional documentation were collected and their analysis began.
At the end of September 2024 a 30% of the survey was completed and the analysis of the documents already collected is finished.
In the last quarter of 2024, 3 documents produced by the Piedmont Endocrine-Diabetological Network were analyzed in terms of scientific appropriateness and the opportunity to induce organizational/clinical changes, relating to the patient with Gestational Diabetes, the management of the elderly patient with Diabetic Retinopathy and the vaccination program in the diabetic patient.
The main output expected is a policy brief on the state of the regional networks on non-time-dependent diseases which will include a comparison on different organisational characteristics and specific recommendations to improve standardisation and quality.
