Task 1.1 includes the following activities:
Brief description of the activities and of the intermediate results: During the reporting period, a careful database mapping was performed to identify those relevant to the task objectives. A database review was started to examine common variables and preliminarily establish harmonization procedures. This activity has been conducted in collaboration with investigators of WP4 to ensure that the extracted variables are suitable for exploration of health and functional trajectories. A set of key databases was identified in collaboration with investigators of WP4. Harmonization procedures and possible machine learning approaches were preliminarily discussed with investigators of the Department of Statistical Sciences at the Università Cattolica del Sacro Cuore, Campus of Milan. Additional possible databases were identified during an ad hoc meeting with investigators of Spoke 3. Variable selection and labeling were started. Discussion is ongoing with partners of WP4 on operationalization of multimorbidity for creation of ad hoc outcome variables. Access has been gained to the Survey of Health, Ageing and Retirement in Europe (SHARE) and the Invecchiare in Chianti (InCHIANTI) study databases. Data dictionaries are being explored to identify the variables of interest. Variable selection and labeling are ongoing for the Sarcopenia and Physical Frailty in Older People: Multicomponent Treatment Strategies (SPRINTT) project database. Investigator meetings were held with researchers of Spoke 3 to share advancements in task activities. In the first reporting period of 2024, research activities focused on the inspection of the databases made available by the Age-It Consortium partners. Information on the data collected in about 30 databases was shared with the task leaders and investigators involved in task 1.1. These include socio-demographic characteristics, clinical and functional data of about 140,000 adults in different healthcare settings (e.g. communities, hospitals, nursing homes, outpatient clinics). Some databases also include biological data. As expected, the available databases differ in many variables. The identification of comparable measures between these different ageing surveys is essential for the cross-sectional analysis of studies required by the task. Harmonisation activities are proceeding starting from physical function and anthropometric measures.
Main policy, industrial and scientific implications: Preliminary results were obtained together with WP4 using a multivariable classification model to predict the incidence of mobility disability in older adults with multimorbidity, frailty and sarcopenia enrolled in the SPRINTT study. The same model will be validated in the LIFE study cohort upon approval of database access.
During the reporting period, we conducted analyses in collaboration with other investigators of WP4 to identify predictors of negative outcomes in multimorbid older adults living with frailty using the SPRINTT database. We built and tested several multivariate models for their ability to identify predictors of negative outcomes in multimorbid older adults living with frailty using available longitudinal databases. The initial results were discussed internally to plan further analyses. As a complementary action, a meeting was held with the Data Management Board of the Working Group on Data Harmonization of Age-IT. Possible strategies for national biological data harmonization were discussed and presented at the annual meeting of Age-IT.
Main policy, industrial and scientific implications: Preliminary results on predictors of negative outcomes in older adults living with frailty. Analyses will be refined and findings will be validated in other longitudinal databases. Strategies for national biological data harmonization and exploitation will be identified and shared with Age-IT investigators.
During the reporting period, multivariate models were built and tested for their ability to identify predictors of response to nonpharmacological treatments in multimorbid older adults living with frailty using available longitudinal databases. Initial results were presented to relevant Age-IT investigators and their feedback was used to conduct additional analyses. Findings were summarized in a manuscript draft to be circulated among Age-IT partners for eventual submission. Data analysis on predictors of negative outcomes in older adults living with frailty is close to being finalized.
Main policy, industrial and scientific implications: Predictors of response to nonpharmacological treatments in multimorbid older adults living with frailty were identified through the analysis of available databases. Results will be reported in a paper to be submitted to a peer-reviewed scientific journal.
During the reporting period, analyses were conducted in harmonized international databases to evaluate the impact of lifestyle interventions on polypharmacy in older adults living with frailty and multimorbidity. The results were discussed with relevant Age-IT investigators and were summarized in a manuscript. The article has been submitted to a top-ranked scientific journal and is currently under review. We also completed the analyses on predictors of negative outcomes in older adults living with frailty. The manuscript reporting the results of our analysis has been finalized and will shortly be submitted to an international scientific journal. Results will also be submitted to NIA to gain access to relevant databases.
Main policy, industrial and scientific implications: The impact of lifestyle interventions on negative outcomes has been assessed in a large and heterogeneous cohort of older adults enrolled in international clinical trials. Results have been summarized in a manuscript submitted to a peer-reviewed scientific journal.
