Innovative, “one health” integrated and personalised approaches during the entire lifecourse as modifiable factors account for 76% of successful ageing determinants. Health promotion and disease prevention cat take advantage of new anti-aging strategies that can support the modification of nutritional and physical activity habits during the lifecourse (education, dissemination, personalized nutritional approach and counseling, physical activity coaching and adapted physical activity, neuropsychological cognitive enhancement, integrated social and health services, health and digital literacy). Furthermore, innovative approaches in the social and psychological domains can contribute to improve individual and collective wellbeing for citizens with different complexity of needs, including chronic and multimorbidity patients. To this purpose, social prescribing can be also integrated.
