Heart failure (HF) is one of the more frequent causes of repeated hospital admission in the older population and is associated to a lower quality of life and frequent need of long-term care and a significant caregiver burden, with caregiver support typically less extended than for cancer.
The Italian ministry of Health recently issued new national directives (DM71/2020) following COVID-19 pandemic in patients with respiratory failure focused to ameliorate the integration between hospital and territorial health assistance and to increase the role of telemedicine in the surveillance of these patients. There are several solutions available for monitoring patients with respiratory failure, but they do not adequately match patient monitoring with caregiver support.
The aim of the present WP will be to develop the first prototipe platform for e-health provision for patients with heart failure.
